How to email membership form

Download the membership form.

Open the form in Microsoft Word, WordPad or WordPerfect.

Fill out the form.

Start your email program and create a new email.

Email address: tsagw@aol.com   Subject: Membership form

Attach the completed form the email and send.

NOTE: Emailing of membership form should only be used when paying membership fee by credit card.  If you plan to send a check, please print the completed form, attach a check and mail to:
